Calf implantation has evolved from a slow but steady development into a much sought-after operation. There is a sizeable portion of the population requesting this procedure for their leg shape improvements.
We use soft, solid implants shaped as the gastrocnemius bellies, which are easier to keep with their shape and positioning.
The actual pockets that have been developed encompass an area going from the midline to the medial fascia insertion on the tibia and inferiorly 2.5 to 5 cm below the lower bellies of the medial and lateral gastrocnemius muscle and laterally over the lateral belly and under the deep fascia.
Calf implantation is performed on both sexes with equal success.
Common patient is the general population, in particular females who have always been ashamed of their skinny legs. Aside from body builder, men have the same desire to obtain shapely legs as do women.
The implantation procedure can be performed while the patient is under local or general anesthesia.
The actual implants are supposed to have the same consistency and feeling as the muscles of the calf. The implantation is made in a defined pocket, and the purpose is to simulate the volume and the shape of the gastrocnemius muscle because it is the muscle that is the most superficial in the posterior calf and the easiest to imitate in its augmentation. These implants are supposed to withstand a certain amount of pressure and trauma, thus remaining dependable for many years in the patient's legs. The author prefers the implants to be as soft as possible.
THE IMPLANTATION
The author has the technique of subfascial implantation, which consists of placement under the deep fascia of the leg and over the gastrocnemius muscle's medial and lateral bellies. The deep fascia of the leg is a solid structure that is able to contain the implant pressure.
CHOICE OF IMPLANTS
Solid, soft silicone implants have been created as soft as possible without losing the cohesion of the implant because an implant that too soft could become friable.
